National Health Insurance Coverage 国民健康保険の給付
When you show your National Health Insurance card (国民健康保険証 kokumin kenkō hokenshō) at a hospital or clinic, you will only be required to pay a percentage of your health care costs. The remainder will be paid by the National Health Insurance program.
Percentage Paid By the Insurance Holder (the percentage paid by you)
- Children under the age of entering compulsory education (children under the age of elementary school)・・・20%
*You can also apply for Children's Medical Support Benefits(子ども医療費助成制度 kodomo iryōhi josei seido) - Persons of elementary school age (compulsory education) through age 69・・・30%
- Persons age 70-74・・・20% (those born on or after April 2nd, 1944)*If your income exceeds a set amount, you will be required to pay 30%

病院などで国民健康保険の保険証を使ったとき、医療費（医者にみてもらうことなどにかかったお金）の割合（％）を自分で払わなければいけません。残りは国民健康保険から払われます。
保険を持っている人（あなた）が払う割合(%)
- 義務教育に当たらない年齢の子ども・・・20%
※子どもの医療費の一部を払う「子ども医療費助成制度」もあります。 - 義務教育に当たる年齢～69歳の人・・・30%
- 70歳～74歳の人・・・20％（1944年4月2日以降生まれの人）※一年に決まった金額以上の所得がある人は30%払わなければいけません。

Support for High Medical Bills 高額医療費（病気を治すためたくさんお金を払うとき）
Even with insurance, if you have to pay more than a decided amount for medical care in one month, you may be eligible for financial support. This amount is called the Individual Payment Limit (自己負担限度額 jikofutan gendo gaku). You must apply for approval. If approved, the amount exceeding the Individual Payment Limit will be paid for by insurance as a High Medical Bill payment (高額医療費 kōgaku iryōhi).
The Individual Payment limit is determined by age (persons under 70 and persons 70 or older) and by income.
After approval, you will be given a Payment Limit Application Approval Certificate (限度額適用認定証 gendogaku tekiyō ninteishō) . You must show this certificate when checking in at a hospital or clinic.

保険があっても、同じ月内に医療費（病気を治すために払うお金）が決まった金額を超えたとき、助かる制度があります。その決まった金額を「自己負担限度額」と言います。申請が必要です。申請が認めた場合、「自己負担限度額」を超えた分は、保険が「高額医療費」として払います。
「自己負担限度額」は年齢（70歳未満と70歳以上の人）と所得によって決められます。
申請が認めた場合、「限度額適用認定証」をもらいます。病院などに行くとき必ず受付で出してください。

High Medical and Nursing Care Bills Assistance 高額医療高額介護合算制度（医療費と介護費を同時にたくさんお金を払うとき）
If at least one member of your household are enrolled in both National Health Insurance and Nursing Insurance and are paying high medical/care bills, you can apply for assistance. An Individual Payment Limit will be applied (for one year from August to July of the next year)

世帯内で国民健康保険と介護保険の両保険を使っていることによって病気や介護のために自分で払うお金の金額が高くなったときは、両保険により1年間（8月から翌年の7月まで）の「自己負担限度額」を超えた分が払われます。

Medical Fee Reimbursement 療養費（払い戻しを受けることができるとき）
Under the following circumstances, you can apply for a refund on a percentage of your medical fees:
- When couldn't show your insurance card because of an emergency
- When you couldn't show your insurance card when receiving treatment for another, unavoidable reason
- When you receive massage, acupuncture, or moxibustion treatment on recommendation of a doctor
- When you use an assistive device (補装具 hosōgu)

こんなときは決まった割合で病院などに払ったお金の払い戻しを受けることができます。
- 急に病気になったことで保険証を病院に出すことができなったとき
- 他のやむを得ない理由で保険証を病院に出すことができなかったとき
- 医者が必要だと言ったマッサージ・はり・きゅうの医療を受けたとき
- 補装具（体を支援する道具）などを使ったとき

Subsidy for Meals During Hospitalization 入院時食事療養費（入院しているときの食事代の支援）
When you are hospitalized, you are required to pay for a portion of your meal fees and insurance will cover the rest.
If you are a member of a household that is exempt from paying residency taxes (住民税非課税世帯 jūminzei hikazei setai) you can apply for a Standard Individual Payment Limit Reduction Approval Certificate (限度額適用・標準負担額減額認定証 gendogaku tekiyō・hyōjun futangaku gengaku ninteishō).

入院をしている間に食べる食事の食事代は、自分で決まった割合（％）を払います。これを「自己負担額」または「標準負担額」と言います。残りの部分は国民健康保険が払います。
住民税を払わなくてもいいと認めている世帯（住民税非課税世帯）の人は、入院中の食事代を安くするため「限度額適用・標準負担減額認定証」をもらうように申請ができます。

- They give birth to a baby (One time allowance for childbirth and childcare 出産育児一時金 shussan ikuji ichijikin)
